Chapter 4 - The Son He Never Knew

The operating room was colder than Lena expected.
Bright lights.
Masked faces.
Metal instruments.
Machines.
Everything moved quickly.
Dominic sat near her head, wearing hospital scrubs over clothes designed for boardrooms and funerals.
His hand never left hers.
Lena stared toward the ceiling.
“I can’t feel my legs.”
“That’s probably intentional.”
She turned her head toward him.
“Was that a joke?”
“I’m trying something new.”
“Don’t.”
“Understood.”
Another minute passed.
Lena could hear doctors talking.
She hated not being able to see.
“What’s happening?”
Dominic looked toward the surgical team.
The doctor spoke calmly.
“We’re almost there.”
Lena tightened her grip on Dominic’s hand.
He leaned closer.
“You’re crushing my fingers.”
“Good.”
“I deserved that.”
“Yes.”
Neither of them smiled.
Then suddenly—
A cry.
Small.
Sharp.
Alive.
Everything stopped.
Lena’s face crumpled.
Dominic froze.
The cry came again.
A nurse smiled.
“Baby boy.”
Lena started sobbing.
“Is he okay?”
“He’s perfect.”
Dominic stared toward the nurse.
For years, men had feared him.
Judges had taken his calls.
Politicians had accepted his money.
Rivals had died because of his decisions.
None of it prepared him for the sight of a seven-pound infant being lifted beneath hospital lights.
His son.
The nurse carried the baby closer.
Dominic stopped breathing.
“He’s healthy,” she said.
Lena turned her head.
“Let me see him.”
They brought the baby to her.
He was wrapped in a white blanket, his tiny face red from crying.
Lena kissed his forehead.
“Hi, baby.”
Dominic stood beside her, unable to move.
Lena looked at him.
“Dominic.”
He blinked.
“What?”
“Come here.”
He moved closer.
The baby opened his eyes for barely a second.
Dominic’s expression broke.
Not dramatically.
No tears fell.
But Lena saw the exact moment the most powerful man she had ever known became completely helpless.
“He has your hair,” she whispered.
Dominic shook his head.
“Poor kid.”
Lena laughed through her tears.
Then her expression turned serious.
“Do you want to hold him?”
Dominic looked at her as if she had asked him to carry explosives.
“I don’t know how.”
“You learn.”
“I’ve never held a newborn.”
“Neither had I five minutes ago.”
The nurse helped position the baby in his arms.
Dominic looked down.
His hands were tattooed.
Scarred.
Hands that had signed orders, handled weapons, and built an empire.
Now they trembled around seven pounds of life.
The baby made a tiny sound.
Dominic immediately looked at the nurse.
“What does that mean?”
The nurse smiled.
“It means he made a sound.”
“Is something wrong?”
“No.”
“Are you sure?”
“Yes.”
Lena watched him.
For the first time in months, she saw the man from the school fundraiser again.
Not the mafia boss.
Not the man in the garage.
The man who remembered coffee orders.
The man who bought her classroom books because she casually mentioned the school budget had been cut.
The man who once spent forty minutes helping a seven-year-old build a cardboard solar system.
Maybe both versions had always been real.
That did not erase what he had done.
But it made the truth more complicated.
Dominic looked at her.
“What’s his name?”
Lena hesitated.
“I hadn’t decided.”
“You had nine months.”
“I was busy hiding from organized crime.”
“Fair.”
She almost smiled.
Then Dominic looked back at the baby.
“What were you considering?”
“Ethan.”
Dominic repeated it quietly.
“Ethan.”
“Do you hate it?”
“No.”
“You look like you hate it.”
“I’m thinking.”
“That’s usually dangerous.”
His mouth moved slightly.
“I like it.”
Lena watched him.
“Ethan Carter.”
Dominic’s expression changed.
There it was.
The surname.
Lena braced herself.
He looked at her for several seconds.
Then nodded.
“Ethan Carter.”
She blinked.
“You’re not going to argue?”
“No.”
“Why?”
“Because you carried him.”
Lena said nothing.
“You protected him when you thought I was the danger,” Dominic continued. “You get to choose his name.”
That was not the response she expected.
Dominic looked down at his son.
“But when he’s older, I’d like him to know who his father is.”
Lena’s body stiffened.
The moment of peace vanished.
“Dominic.”
“I’m not asking tonight.”
“You are not taking him.”
His head snapped toward her.
“I would never take him from you.”
“You say that now.”
“I’m saying it forever.”
Lena looked unconvinced.
Dominic handed Ethan carefully back to the nurse.
Then he moved beside Lena.
“I need you to hear me.”
She stared at him.
“I searched for you because I needed to know you were alive. When I learned you were pregnant, I wanted to find you even more.”
Lena’s eyes hardened.
“I know.”
“But I didn’t touch you.”
“You watched me.”
“Yes.”
“That’s not normal.”
“I’m not pretending it is.”
“Then why?”
“Because there are people who would use you to get to me.”
Lena went pale.
“That’s exactly why I ran.”
“I know.”
“No, you don’t.”
“I do now.”
The doctor finished the procedure while they spoke quietly.
Dominic continued.
“You were right about one thing.”
“What?”
“My world is dangerous.”
Lena laughed bitterly.
“That’s a dramatic understatement.”
“I can’t change what I was born into.”
“You became the boss.”
“Yes.”
“You chose that.”
“Yes.”
He did not hide from the truth.
That unsettled her more than excuses would have.
Dominic lowered his voice.
“But Ethan does not have to inherit it.”
Lena studied him.
“You expect me to believe that?”
“I expect you to judge what I do next.”
Two hours later, Lena rested in recovery with Ethan sleeping against her chest.
Dominic stood near the window.
He had not left.
He had also not touched Lena again without permission.
Rachel entered carrying coffee.
She stopped when she saw him.
“You’re still here.”
Dominic nodded.
“You sound disappointed.”
“I’m deciding.”
Rachel handed Lena a cup.
Then looked at Dominic.
“Your security guys are frightening the nurses.”
Dominic frowned.
“They’re downstairs.”
“One is six-foot-six and looks like he eats furniture.”
“That’s Carlo.”
“I don’t care what his name is. Tell him to smile.”
“He doesn’t know how.”
“Teach him.”
Dominic pulled out his phone.
Lena stared at them.
“This is surreal.”
Rachel looked at Ethan.
Then smiled.
“Oh.”
She moved closer.
“Hello, handsome.”
Dominic watched carefully as Rachel held him.
Rachel noticed.
“You realize I helped hide this baby from you for eight months.”
“I’m aware.”
“And you’re okay with me holding him?”
“You protected him.”
Rachel looked surprised.
Dominic added, “And her.”
Rachel handed Ethan back to Lena.
“Well. That was unexpectedly reasonable.”
Dominic put his phone away.
“Don’t get used to it.”
Lena looked down at her son.
For several minutes, everything felt almost normal.
Then Vincent entered without knocking.
The moment Dominic saw his face, he knew.
“What happened?”
Vincent looked at Lena, then Rachel.
“We should talk outside.”
Dominic did not move.
“Say it.”
Vincent hesitated.
“Someone leaked the hospital location.”
Lena’s blood turned cold.
Dominic’s expression changed instantly.
The tenderness disappeared.
The man from the parking garage returned.
“Who?”
“We don’t know yet.”
“What does that mean?”
“It means word is moving through Chicago that Lena gave birth.”
Rachel stepped closer to the bed.
Lena held Ethan tighter.
“How would anyone know?”
Dominic looked at Vincent.
“Who knows she’s here?”
“Our immediate people. Hospital staff. Whoever followed Rachel.”
Vincent shook his head.
“Our team didn’t.”
Dominic’s jaw tightened.
Lena stared at him.
“What happens now?”
Dominic walked toward her bed.
“We move you.”
“No.”
“Lena.”
“No. I just had surgery.”
“I’m not taking you out tonight unless necessary.”
“Then what are you saying?”
“This floor gets secured.”
“This is a hospital, Dominic.”
“I know.”
“You cannot take over a hospital.”
Vincent cleared his throat.
“He kind of can.”
Lena glared at him.
Vincent raised both hands.
“Not helping. Sorry.”
Dominic leaned closer.
“Listen to me.”
“No. You listen.”
Lena’s voice shook with anger.
“This is what I was afraid of. Ethan has been alive for two hours and your world is already at his door.”
Dominic went silent.
“You promised he wouldn’t inherit this.”
“I said he doesn’t have to.”
“Well, it looks like your enemies disagree.”
The accusation hit.
Dominic looked at his son.
Then at Lena.
“You’re right.”
That answer stopped her.
Dominic turned to Vincent.
“Find out who leaked it.”
Vincent nodded.
“And Dominic?”
“What?”
“If this is who I think it is, we have bigger problems.”
Dominic’s eyes narrowed.
“Who?”
Vincent looked toward Lena again.
“Salvatore Bellini.”
Dominic’s face became stone.
Rachel whispered, “Who is that?”
Lena did not want the answer.
Vincent gave it anyway.
“Someone who has been waiting three years for a way to hurt Dominic.”
Lena clutched Ethan tighter.
Dominic stepped beside her.
“No one comes near this room.”
Lena looked up.
“And if they do?”
Dominic’s eyes went cold.
“Then they learn why they should have stayed away.”
